5 Simple Steps to Receiving your Breast Pump At No Cost through your UCare Minnesota

Step 1: Complete the Insurance Eligibility Form 

Make sure to include your full insurance member ID, including any prefix characters. This helps us quickly verify your eligibility. 

  • If you include your doctor’s phone number on the form, we'll reach out to their office directly.  

  • For your convenience, we offer an option to upload a copy of your insurance card. This ensures that we have all the necessary details to process your request smoothly. 

Step 2: Insurance Verification

Next, we'll contact your provider to request a prescription for a breast pump on your behalf. Before we can order your breast pump, we must have your prescription.

If you have UCare Minnesota, you qualify for a breast pump during each pregnancy you have. To start the process of receiving a breast pump to your home, first fill out our Insurance Eligibility Form and we can handle the rest, including confirming your insurance coverage. 

Minnesota UCare insurance coverage requires a prescription from a UCare authorized provider. To get your prescription signed, you can simply print out our prescription here and bring it to your next doctor’s appointment. We are happy to get your prescription faxed to us or you are able to upload your signed prescription to our website.  

UCare Minnesota covers one breast pump per pregnancy. Approved pumps come in a variety of styles to suit mom’s unique needs.
